Ambulance Victoria will undertake further assessment regarding transfer to hospital and may sometimes access other services such as Virtual ED to ensure the most appropriate care for your patient.

The Community Care team will remain available for GPs to discuss clinical care of patients. This is especially the case when there are clinical dilemmas, when a patient is likely to need admission or definitely needs remdesivir (eg transplant patient, active chemotherapy patient).

Post Covid Recovery

The formal Post-Covid Recovery Program is closed. ACCESS will still process requests for clients with long Covid symptoms. Rather than booking them to a program, patients will be given appointments with individual allied health clinicians depending on their presentation and initial needs assessment.

Coronavirus
- The South East COVID Pathway is operated by Sandringham Ambulatory and Community Care (SACC) in partnership with the South East Metro Health Service Partnership (SEMHSP). The pathway was developed to support GPs in managing COVID-19 positive patients in the community and provides advice and streamlined escalation of care of patients at risk and in the event of deterioration. SACC provide patient monitoring and will handover vulnerable patients with ongoing concerns needing follow up and leaving the program, to GPs
